Permission to Participate Form

If you have paid for and filed an application for graduation with the Registrar's office and have been informed that you are lacking requirements for completion of your degree, please click the link below to access the Permission to Participate Form. This form is for students who lack 6 credit hours or less for completion of ALL degree requirements.

Once you obtain the necessary signatures, please bring the completed form to the Office of Academic Affairs in Colston Administration Building, Room 221. This form must be received by the Office of Academic Affairs no later than 3:00 p.m. on the Friday before the Commencement Ceremony.

If there are any concerns about your degree requirements, please contact your academic advisor or your department chair.
